摘要

以頭痛為主要症狀之鼻竇疾病，常易偏向於神經學方面之病因探討，而導致延誤診斷及適當之治療。除了急、慢性發炎反應外，鼻竇內血腫塊形成後所造成之內壓變化及黏膜刺激也會引起強烈之頭痛。民國85年4月間1名56歲女性因頭痛急診。傳統顱部X光檢查及電腦斷層授描發左側額竇有一4×2 cm大小之軟組織，手術取出後，經組織病理學診斷為額竇黏膜下血腫塊，術後病人症狀完全消失，因屬罕見病例，特提出報告。

A paranasal sinus disease complicated with headache may be misdiagnosed as a neurological disease, which may result in delay of appropriate management on the underlying sinus disease. Not only inflame-matory but also non-inflammatory diseases of the paranasal sinuse, e. g. the submu-cosal hematoma ov frontal sinus, can the pressure change and irritation of the sinus mucosa. We reported a case of submucosal hematoma of the frontal sinus with initial symptom of headache. Through and osteoplastic approach, the hematoma was successfully removed and proven histo-logically. Besides, the mechanism of paranasal pain caused by the frontal sinus hematoma was discussed.
