Neuro-otolgical Findings in Parkinson's Disease

摘要


背景:巴金森病人偶會有頭暈、眩暈現象,由於機轉不明,故進行本研究。 方法:自1993年7月至1997年6月,14名罹患眩暈之巴金森病人,接受一系列之聽力平衡功能檢查,包括球運動檢查及重心動搖檢查。 結果:眼球運動檢查出現異常者有13例(93%),其中跳視測驗異常者12例(86%),追蹤測驗異常者10例(71%),視運動眼振檢查異常者8例(57%)。重心動搖查顯示不論開眼或閉眼,其姿勢控制皆不良。 結論:巴金森病人之眩暈及姿勢控制的障礙,其機轉主要在視運動路徑,及與視覺相關之姿勢控制路徑上發生病變所致。

Background: Patients with Parkinson’s disease have dizziness or vertigo occasion-ally. This study is aimed to clarify the mechanism. Methods: From July 1993 to June 1997, fourteen patients of Parkinson’s disease with dizziness or vertigo were subjected to a battery of neuro-otological tests. Seven of them performed posturography. Results: Thirteen patients had oculomotor abnormalities (93%). Twelve (86%) showed abnormalities in saccade test, ten (71%) in pursuit test, eight (57%) in optoknetic nystagmus test. Posturography revealed poor postural control whether with eyes open or closed. Conclusion: Involvement of oculomotor pathway and visuopostural loop is responsible for vertigo and poor postural control of patients with Parkinson’s disease.
