The reconstruction of soft tissue defects of Achilles tendon, malleoli, and posterior heel has been a great challenge to plastic surgeons. Although there are many local flaps, distal flaps, and free flaps to be chosen, all of them have its indications and limitations. The lateral calcaneal artery skin flap, which including lateral calcaneal artery, lesser saphenous vein and sural nerve, can provide a sensory, nonbulky, versatile flap with minlmal donor site morbidity. The proudure of this flap, either transposition or island, is simple and effective. Careful pre-operative vascular evalution of foot with Doppler flowmetry or angiogram is indicated for patients who are systemically compromised with trauma, diabetes or peipheral vascular diseases. So it is a good alternative method for soft tissue reconstruction around posterior heel. We present four cases to share the experiences of lateral calcaneal artery skin flap with you.
