Superficial temporal fascial free flap is an excellent choice for dorsal foot reconstruction. It is thin and pliable. It allows free temdons glide beneath the fascial flap. The donor site is inconspicous and morbidity is minimal. A free fascial flap with split thickness skin graft were applied to a patient with severe dorsal foot avulsion as immediate reconstruction six hours following the injury. The wound healed uneventfully and the contour of dorsal foot was maintained. Toes extension was good. The patient was discharged two weeks after the operation. In conclusion. We suggest that the free superficial temporal fascia flap is the ”flap of choica” for foot dorsum reconstruction.
