摘要

高雄醫學院重建整形外科自1985年10月至1991年9月總共經歷60例腮腺腫瘤。其中男性35位，女性25位，平均44.5歲。良性腫瘤佔50例，惡性腫瘤佔10例。良性腫瘤中之男女比例相等；惡性腫瘤以男性居多9:1。良性腫瘤以混合性腫瘤(Mixed tumor)28例最多，Warthin氏腫瘤(Warthin's tumor)8例居次。惡性腫瘤則以類腺囊癌(Adenoid cystic carcinoma)最多。良性腫瘤以無痛性腫塊最多，共有42例；而惡性腫瘤則以固定腫塊較多，有7例。惡性腫瘤中有局部淋巴或肌肉轉移的有5例；良性腫瘤中有局部淋巴腫大的佔11例。術後有暫時性顏面神經損傷者，有13例；有Frey氏症候羣(Frey's syndrome)的有4例。
本篇回顧顯示腮腺腫瘤大部份是良性，而良性腫瘤以混合性腫瘤居多。良性腫瘤的病人的主訴以無痛性腫塊為主。惡性腫瘤以男性居多。固定腫塊的腮腺腫瘤有高比例的惡性傾向。

Sixty patients treated for tumors of the parotid gland have been analyzed retrospectively from Oct 1985 to Sep. 1991. There were 35 males and 25 females. The average age of patients were 44.5 years. Fifty of 60 patients were benign tumors and the other ten were malignant tumors. Benign tumors were equally common in males and famailes. Malignant tumors were nine times more common in males. Mixed tumor was the commonest benign tumor accounting for twenty-eight of 50 patients. The commonest malignant tumor in our series was adenoid cystic carcinoma. Forty-two of 50 patients with benign tumors presented with a painless lump. Among ten patients with malignant tumors, seven had fixed masses; five had lymphatic metastasis or muscular invasion. Eleven of 50 patients with benign tumors had local lymph node enlargement Thirteen patients (21.7%) had temporary facial palsy and four patients (6.7%) had Frey's syndrome after operation.
It is concluded from this study that most parotid tumors are benign and most are mixed tumors. A painless lump is the most consistent presenting sympton among patients with benign tumors. Malignant tumors are more common in males. Fixed parotid mass in a patient is at high risk of malignancy.
