Subluxation of the First Carpometacarpal Joint-A Case Report

拇指腕掌關節半脫位-病例報告

摘要


拇指腕掌關節半脫位,係由於關節囊韌帶斷裂或Bennett氏骨折癒合不良而造成該關節向掌背側脫位。臨床上會造成腕掌關節疼痛及拇指捏力(pinch power)減弱。對於較罕見的急性外傷後、輕微的半脫位(acute minor suduxation),常因症狀不明顯且X光檢查並無顯著的移位而容易被忽略;不適當治療的結果,將造成慢性的疼痛及活動力減弱。 本病例報告為慢性外傷後拇指腕掌關節半脫位,於手術復位時發現關節面完整,未有骨折的徵象,經利用撓側屈腕肌腱來重建掌側關節囊韌帶而得到良好之結果。

Subluxation of the first carpometacarpal joint is relatively uncommon, However, when it does occur, it causes posterior instability of the basal joint either as a result of the tearing of the capsular ligament or as a secondary effect of malunion of Bennett's fracture. This is a case of a post-traumatic subluxation of the first carpometacarpal joint with intact articular surface and it was treated by reconstruction of volar ligament using flexor carpi radialis tendon sling obtainlng a satisfactory result. This kind of injury should be detected and treated in the acute stage.
