Reconstruction of the Lower Abdominal Wall Defect with Rectus Abdominis Musculo-fascial Flap in Adult Exstrophy of the Bladder- A Case Report
利用腹直肌肌筋瓣重建成人型膀胱外翻之下腹壁缺損－一病例報告
摘要
一位先天性膀胱外翻一尿道上裂的成人男性，從小一直被誤認為女性。於小孩時手術失敗後，未再做適當及完整的手術處置。因膀胱長久外翻，已有黏膜異生現象。病患於接受膀胱切除術、尿路分流、陰莖延長術及利用腹直肌肌筋瓣來重建下腹壁缺損，術後恢復下腹壁的完整性。腹直肌肌筋瓣除了擁有大面積及高韌性等特點外，兼具手術容易、時間短及同一手術傷口等有利因素，實為此一類型重建的良好選擇。

A rare case of adult exstrophy of urinary bladder with squamous metaplasia is presented. Total cystectomy, urinary diversion, penile lengthening and reconstruction of lower abdominal wall defect using a rectus abdominis musculo-fascial flap were done. For those complex problems in adult exstrophies in which preservation of the bladders is not feasible, this flap provides a versatile and dependable reconstructive option for the lower abdominal wall defect.
