使用紅外線熱攝影在燒傷傷口之評估初期報告

Use of Thermography in Burn Wound Assessment

摘要


燒燙傷病人的癒後結果和初步的傷口處置有密切的關係,近年來醫學界試用各種先進的儀器測量燒燙傷傷口,但又限於機器的昂貴或使用時病人的不適而不能廣泛推行;有見於此國泰醫院燒傷中心從1996年開使始用紅外線熱攝影在燒傷傷口的評估,期望未來可以利用這種非接觸性的診斷方式,在臨床上因而改善病人的癒後結果及縮短住院天數。 從一月至四月共進行七位病人的傷口評估,分別有二度及三度的化學、燒傷及燙傷,面積5%~36%不等;初步的心得是:一、紅外線熱攝影在燒傷初期即可迅速分辨二度及三度傷口。二、在傷口感染時紅外線熱攝影有明顯的表現可助於醫師清創時的決定。三、紅外線熱攝影是非接觸性診斷方法,病人可以免於疼痛的恐慌。至於是否可以粗略判斷傷口癒合的時間須要再進一步的病人採樣與統計。

The evaluation of initial wound depth following burn injury is a challenging problem in deciding the correct regimen which will have the potential impact on the outcome of wound healing and scar formation. Thermography is a noncontact method of mapping the temperature of a surface and when applied to man provides a thermal map of the skin. We sought to determine if a thermography could be adapted to provide an objective measurement of the burn wound. Cathay General Hospital Burn Unit has evaluated 7 patients with second to third degree burn from January to April, 1996 by using thermography on their burn wound and recorded their progress. Preliminary observations are: 1. Thermography is a valuable tool in differentiating second and third degree burns immediately upon receipt of the patient. 2. Thermography is a noncontact examination which can spare burn patients from unnecessary fear and pain. 3. Thermography can easily identify areass of infection.
