Single Pedicle Microsurgical Transfers of Combined Serratus Anterior, Latissimus Dorsi and Cutaneous Maximus Muscle Flap in Rats

在老鼠合併前鋸肌、闊背肌及皮大肌單一血管莖皮瓣之顯微移植

摘要


近年來在組織傷害以及皮瓣存活率的研究方面,已有許多實驗模型可以拿來作生化分析和活體觀測。理想的模型應該具備許多特點,包括:與臨床應用相近;技術上易於複製有;豐富的檢體量;能夠清楚的判別皮瓣存活程度;可靠的檢體來源;以及較低的手術失敗率。 本研究設計一個模型:在S-D老鼠以單一血管莖結合前鋸肌、闊背肌和大肌的皮瓣,剝離至腋動脈做爲血管莖,將其轉移至股動脈上,其後維持灌注六小時。在七雙老鼠的實驗中,測量皮瓣的尺寸、純肌肉部份的重量、血管莖的長度及直徑和操作時間。結果證明這個皮瓣具備了較長的血管莖、較大的血管直徑以及容易操作剝離的特性。本研究結果認爲在某些特定實驗用途上,這是一個理想皮瓣。

In the studies focused on tissue injury and flap viability, various kinds of models have been used for biochemical analysis or in vivo observation in recent years. An ideal model requires the characteristics of analogy to clinical use, reproducible results by simple skill, abundant specimen volume, non-ambiguous flap survival, reliable tissue source and low intra-operative death or failure. We designed a single pedicle combined serratus anterior, latissimus dorsi and cutaneous maximus muscles flap that met the conditions previously mentioned in the laboratory need. Spraque-Dawley rats were used for the experiment. The combined muscle flap was dissected to axillary vessels level. It was then transferred to the femoral vessels under micro-surgical technique and reperfused for six hours after ischemia. Dimensions of flap, total weight of pure muscles mass, pedicle length, vascular pedicle diameter and dissection time were measured. This flap provided abundant muscle volume for serial tests and also revealed long in pedicle length, large in pedicle caliber, and easy to harvest and anastomose for a non-skilled operator. The results proved that this flap is a very good option in the laboratory use.
