Reconstruction of Orbital and Maxillary Defects Using Free Rectus Abdominis Myocutaneous Flap with Vascularized Costal Cartilages-A Case Report
利用腹直肌自由肌皮瓣術併帶血管莖肋軟骨重建眼窩及上顎缺損
摘要
對於顱顏惡性腫瘤廣泛切除後所造成之缺損的重建，迄今仍是一困難的手術。其包括之硬及軟組織缺損往往會造成臉部輪廓塌陷變形，要重建其外觀則同時需要一硬組織來重建其骨頭缺損，及一軟組織來填塞其軟組織之缺損；至目前仍無一塊皮瓣可達成此需求。1995年，日本醫師Yamamoto描述其實驗結果，證實能於取自由腹直肌皮瓣之同時，也帶一塊具血管之肋骨。兩年後，他再度發表兩病例報告，由其結果我們認為此一自由瓣為重建此類缺損之可選方法。因此我們在此提出一病例報告。

Reconstruction of craniofacial defects following tumor ablation is a difficult problem. The hard and soft tissue deficiencies will make collpase and deformity of the facial contour. Restoring the contour usually needs a hard tissue to maintain the skeleton and a bulk soft tissue to tamponade the soft tissue defect. No single flap fits the requirement. In 1995, Dr. Yamamoto described the possibility of harvesting a vascularized rib combined with free rectus abdominis myocutaneous flap. This seemed to be a good choice for reconstruction of such defect. In this paper, we present one of our experiences.
