Expandable Metallic Esophageal Stent: Preliminary Report of a Lab-Built Process, in Vitro Expansile Stiffness Test, and in Vivo Placement in a Pig

可擴張金屬食道支架:初報一種自製程序、體外支撐力測試及活體豬之食道置放

摘要


食道癌是臺灣地區常見的疾病。由於産病早期缺乏明顯症狀,直到阻塞症狀出現,通常已太遲而無法開刀切除。雖然目前有放射治療、化學治療、雷射治療等方法來改善這些症狀,仍缺乏快速有效的治療辦法。在此研究中,我們設計出一種簡單、經濟、有彈性的製作金屬食道Z型支架的程序,幷以數學模式决定編織之幾何參數。製作出來的支架以不同的壓迫力及壓迫範圍來决定其在體外之支撑力,幷以此和同型商品比較,發現兩者性質相當且支撑力和壓迫範圍之平方根成反比。自製支架可成功置入活體豬食道,幷維持至少兩周之適用性,也證明利用此自行發展之製造程序及推演出之支架參數,將可因應不同外形及長度之食道瘤,簡單快速地自製便宜之食道支架。

Esophageal cancer is a common disease in Taiwan. Although asymptomatic in the early stage, it is usually too late to be treated by curative operation once patient present with symptoms, There is still lack of effective treatment to relieve the symptoms in spite that multiple therapeutic modalities, such as radiotherapy, chemotherapy, and laser ablation, etc. are available. In this study, we designed a simple flexible, and economic lab-built process for metallic esophageal Z-stent, A”V”-shaped model, the basic component of Z-stent, was used to determine the parameters for weaving, A method to measure the expansile stiffness of Z-stent was approximately in reverse relation to the square root of compression length. Our lab-built stent was compatible in expansile stiffness and homogeneity with the commercial Z-stent. The lab-built stent was inserted into the esophagus of a pig successfully with short-term applicability. Our developed Z-stent-manufacturing process with adjustable parameters has potentials to make stents economically and quickly adapting esophageal tumors with different sizes.
