A Randomized Controlled Study of Triglycyl-Vasopressin and Vasopressin Plus Nitroglycerin in the Control of Acute Esophageal Variceal Hemorrhage

In a randomized controlled trial, the effect of triglycyl-vasopressin (glypressin) was compared with vasopressin plus sublingual nitroglycerin in 45 cirrhotic patients with bleeding esophageal varices. The two groups of patients were similarly matched for all parameters. Bleeding was controlled initiallyin52% (11/21) and 58% (14/24) of the patients treated with glypressin and vasopressin plus nitroglycerin, respectively (p>0.05). Complete control of bleeding during 24 hr of drug infusion was achieved in only 4(19%) patients receiving glypressin and in 8 (33%) of those treated with vasopressin plus nitroglycerin (p>0.05). Complications associated with drug infusion were significantly lower in the glypressin than in the vasopressin plus nitroglycerin groups (2/21vs 10/24, p<0.0l). Mortality in relation to variceal bleeding and transfusion requirements were similar in the two groups. This study shows that glypressin alone is as effective as vasopressin plus nitroglycerin in controlling variceal bleeding. Glypressin has fewer complications than vasopressin plus nitroglycerin.
