止吐劑所引起之下顎運動失調－病例報告
Antiemetic Induced Motor Disorder of the Mandible-A Case Report
摘要
引起下颚運動失調的原因有很多，色括局部肌肉痙攣中樞運動神經系統異常所引起，其中某些藥物（例如止吐劑等）也可能引起病忠有運動失調的問題。在臨床上，癌症患者在接受化學治療時所引起噁心、嘔吐等症狀經常使用止吐劑(Metoclopramide)來改善。此類止吐藥物可引起錐體外症候群而造成全身不自主的運動，這種不預期的併發症常使醫護人員措手不及。本病例報告爲一接受化學治療之骨肉癌忠者因使用止吐劑Metoclopramide而引起之下顎不自主運動，經診斷爲由Metoclopramide所引起之錐體外症候群，並且在給予抗帕金森症劑Biperiden之後症狀即刻獲得改善。因這種藥物造成下顎不自主運動的合併症相當罕見，故特此提出討論。

Motor disorder of the mandible may be caused by a variety of reasons, including local muscle spasm and central nervous system abnormalities. Some drugs (such as Metoclopramide) may also cause movement disorders. Clinically this drug commonly used for treatment of chemotherapy-induced nausea and vomiting may cause extrapyramidal reactions that result in body involuntary movement. Such complications often lead to misdiagnosis by clinicians. We report a case which was an osteosarcoma patient receiving chemotherapy, and involuntary movement of the jaw occurred after injection of metoclopramide. Extrapyramidal reaction was impressed and the symptom subsided after administration of Biperiden. The complications of drug-induced involuntary movements of the jaw are rare, it is hereby discussed.
