治療上顎囊腫感染合併上顎竇炎－病例報告

上顎囊腫大多因繼發性感染而發現，手術摘除爲主要處理方式。若患者合併有上顎竇炎，治療將更形複雜。本病例因左臉長期反覆腫脹疼痛，經環口X光攝影及電腦斷層影像檢查，發現左側上顎有一類囊腫之病灶合併左側單側上顎竇炎。感染控制之後合併手術治療，術中發現此病灶與上顎竇無相通，以手術摘除上顎囊腫，並以功能性內視鏡鼻竇手術處理上顎竇炎。術後，囊腫及上顎竇炎均得到成功的治療。本病例爲簡單囊腫感染合併複雜的單側上顎竇炎，傳統上面對上顎病灶合併上顎竇炎，常習慣以柯特威爾－路克氏手術摘除病灶，同時作引流治療。本病例對鼻竇炎採用內視鏡引流，避免發生口竇相通的後遺症，提供口腔顎面外科醫師參考。

Both infection control and surgical removal are the major management of infected cyst. Here, we had presented a complicated case which had a simple maxillary infected cyst accompanied with a unilateral maxillary sinusitis. Taking history, the patient had left facial pain for 6 years at least. The patient also had running nose and nasal congestion. Radiographic examinations had revealed a large radiolucent lesion at left maxilla, the sinus also presented cloudy-like shadow. The computed tomography had shown a large, well-defined, and cyst-like lesion at left maxilla, it also revealed homogenous fluid-like contents over left maxillary sinus. Then, infected cyst accompanied with sinusitis was impressed. Finally, the cyst was removed by surgical enucleation and the sinusitis was drained smoothly by FESS instead of Caldwell-Luc operation. All symptoms subsided after operation and patient was under stable and healthy condition. This successful case provides management experiences for clinical application.
