Retention and Marginal Adaptation of Luting Agents on Dental Implant-Supported Prostheses

摘要


背景:此實驗旨在研究七種不同的黏合劑應用於牙科植體支持性牙套其固位力強度之評估與比較。方法:35個鑄造好的牙套由7組不同的黏合劑來黏著於35根植體支臺牙柱上。7組不同的黏合劑其中包括磷酸鋅黏合劑、樹脂黏合劑、氧化鋅丁香油酚基底臨時性黏合劑、氧化鋅基底臨時性黏合劑、丙烯酸酯臨時性黏合劑及樹脂臨時性黏合劑等。在應用咬合模擬機及溫度轉換機來模仿口腔環境及溫度後再測試黏合劑固位力之強度。One-Way ANOVA及Duncan's Multiple Range Analysis用來評估與比較各黏合劑之固位力。結果:每一組黏合劑固位力強度的平均值及標準差分別為:磷酸鋅黏合劑(Zinc phos-phate):0.731 MPa(0.139);樹脂黏合劑(Maxcem):1.24MPa(0.242);氧化鋅丁香油酚基底臨時性黏合劑(Temp Bond):0.182 MPa(0.077);氧化鋅基底臨時性黏合劑(Temp Bond NE):0.204 MPa(0.042);丙烯酸酯臨時性黏合劑(ImProv):0.195 MPa(0.102)氧化鋅丁香油酚基底臨時性黏合劑(Opotow):0.159 MPa(0.033)和樹脂基底臨時性黏合劑(Premier):0.226 MPa(0.074)。結論:樹脂黏合劑(Macem)相較於其他的黏合劑具有最強的固位力而永久性黏合劑的固位力都比臨時黏性黏合劑要強。氧化鋅丁香油酚基底臨時性黏合劑(Opotow)具有最弱的固位力,即使它與其他的臨時性黏合劑在統計上沒有顯著性的差異。

Purpose: Selecting the appropriate luting agent is critical to the long-tem efficacy of cement-retained implant prostheses, in part because some luting agents may prevent retrieval of prostheses for future maintenance. At the same time, a strong marginal seal will prevent marginal leakage that can result in toxin formation and inflammation around implant fixtures. The present study evaluated the seal strength and marginal leakage of 7 different luting agents used with implant abutment/analog assemblies. Materials and methods: Of the agents tested, two were definitive and five were provi-sional cements. The definitive cements were Maxcem resin cement and zinc phosphate cement. The provisional cements included three zinc-oxide eugenol and noneugenol cements (namely, Opotow, Temp Bond and Temp Bond Noneugenol), one acrylic/urethane-based cement (ImProv), and one resin-based cement (premier). Thirty-five Steri-Oss implant abutment/analog assemblies and cast superstructures were used to test these seven cements (five per cement). Following cementation of the superstructures onto the implant abutments, each specimen was subjected to 1,000 cycles on a thermocycling machine (0˚C-55˚C) and 100,000 cycles on a chewing machine (75N). The cement failure stress and marginal leakage were tested for each specimen. The data were analyzed with one-way ANOV A, Duncan's Multiple Range Analysis and Kruskal-Wallis test. Results: The cement failure stress mean of the Maxcem resin cement was significantly higher than that of the other cements (P<.0001), and the mean marginal leakage scores for ImProv, Optow, and Premier were significantly lower than those of the other cements (p =.005). Conclusions: The definitive cements exhibited much stronger retention than the provisional cements. However, there were no significant differences in retentive properties among the five provisional cements themselves. As for marginal leakage, ImProv and Premier showed the least marginal leakage, while Temp Bond and Temp Bond Noneugenol had the highest leakage among the luting agents tested.
