Using Resonance Frequency Analysis to Study Factors Affecting Maxillary Dental Implant Stability

摘要


目的:為了探討影響上顎植體穩定度的因素,使用共振頻率分析法來分析手術術式、骨質條件、植體直徑、植體長度與上顎植體穩定度的相關性。材料與方法:本實驗收集39位病人共55支Straumann SLA植體,於手術過程中測得植體的原始植體穩定商數(直徑3.3mm,簡稱ISQ_0)和初級植體穩定商數(直徑4.1mm或4.8mm或TE,簡稱ISQ_1),經三個月手術恢復期,於贗復物製作前,測得次級植體穩定商數(簡稱ISQ_2),進行統計分析,獲得初期植體穩定商數差(簡稱ISQ_(1-0))和次級植體穩定商數差(簡稱ISQ_(2-1))。結果:上顎植體的初級植體穩定商數差(ISQ_(1-0))與植體直徑在統計學上有顯著差異(p=0.043),次級植體商數差(ISQ_(2-1))與骨質在統計學上有顯著差異(p=0.000)。雖然手術術式與初級穩定商數差(ISQ_(1-0))和次級穩定商數差(ISQ_(2-1))在統計學上並無顯著差異,但使用骨鑿成形術組的ISQ增加值均比傳統組來得多。植體長度和ISQ_(1-0)、ISQ_(2-1)在統計學上並無顯著差異,與之前大部分研究結果類似。結論:影響上顎植體穩定度的因子中,植體直徑和骨質影響較大,骨鑿成形術這項手術術式對增加植體穩定度也有影響。

Purpose: The purpose of the study is to evaluate the factors that affect implant stability on maxilla. Resonance frequency analysis (RFA) is used to analyze the correlations between implant stability and factors such as insertion technique, bone quality, implant diameter, and length of implant.Materials and Methods: A total of 55 Straumann SLA® implants were inserted in the maxilla of 39 patients. The stability of each implant was examined at placement and 3 months after the surgery using RFA. The stability of the implant is measured by implant stability quotient [ISQ]. For the experiment, ISQ_0 will represent ISQ for implant with 3.3 diameters; ISQ_1 will represent ISQ for implant with 4.1mm or 4.8mm diameter; ISQ_2 will represent the secondary stability 3 months after implant placement. The study will analyze the primary ISQ difference between different implants (ISQ_1-0) and secondary ISQ difference before and after the implant placement (ISQ_2-1).Results: The primary ISQ difference (ISQ_1-0) between implants with different diameters in the maxilla is statistically significant (p=0.043). Secondary ISQ difference (ISQ_2-1) and bone quality is statistically significant (p=0.000). Although statistically there is no difference between surgical techniques, ISQ_1-0 and ISQ_2-1, surgery results from osteotome technique show higher ISQ value than traditional technique. There is no significant difference between length of implant and ISQ_1-0 and ISQ_2-1 and the result is consistent compared to other similar established studies.Conclusion: Among the factors discussed in this experiment, implant diameters, and quality of the bone have more significant effects on maxillary implant stability. Implant surgery with osteotome technique increase the stability of the implant in maxilla.
