Oral Health-related Quality of Life of AII-on-4 Concept Immediate Functional Prostheses-A Preliminary Study

摘要

引言：本篇前瞻性研究目的是希望能評估台灣病患接受All-on-4立即負荷植牙之治療不同階段對口腔健康相關生活品質（OHRQoL）的影響，以及討論可能會提升口腔健康相關生活品質的變數。方法：受訪者爲20位自2014年至2017年在高雄長庚醫院牙科接受單顎或雙顎All-on-4立即負荷植牙之病患，病患在植牙手術後5至10天内裝戴臨時性贋復物並立即負荷，在3至6個月後裝戴長期性贋復物。受訪者會在手術前(T0)、臨時贋復物後三個月（T1）、長期性贋復物後三個月（T2）三個時間點給予口腔健康衝量表（OHIP-14T)問卷訪談，OHIP-14T分數越低表示OHRQoL越佳。完成量表以雙樣本中位數差異檢定分析。結果：整體0HIF-14T總分在T0、T1及T2都有具統計意義的顯著下降(p＜ 0.05)。年紀較長(≥50y/o)病患、女性、戴過可撤式義齒、只做下顎重建及有顴骨植體等因素有OHIP-14T總分較低的趨勢，無抽菸習癖其OHIP-14T總分顯著小於有抽菸病患，而長期性贋復物設計對OHIP-14T無影響。 結論：以All-on-4立即負荷植牙治療不論在臨時贋復物階段或長期性贋復物階段都有顯著提升口腔健康相關生活品質。

Introduction: The aim of this prospective clinical study was to evaluate the longitudinal changes in oral health-related quality of life (OHRQoL) attributable to All-on-4 concept immediate functional prostheses in Taiwanese patients. Methods: Twenty patients who underwent placement of four to six endosteal dental implants on the basis of the All-on-4 treatment concept in the edentulous maxilla / mandible or both jaws at the Dental Department of Kaohsiung CGMH were included in the study. Patients were immediate loaded with acrylic interim prostheses and definitive prostheses were replaced after 3-6 months with titanium framework reinforced. The subjects completed the questionnaires including Taiwanese short-form of the Oral Health Impact Profile (OHIP- 14T) at baseline (TO), 3 month after the interim prostheses placement (T1), and 3 month (T2) after the definitive prostheses placement. Complete data were analyzed with the Wilcoxon signed-rank test for comparisons of TO with T1, and T2. Then, Bonferroni correction was applied. Results: The total OHIP-14T scores significantly reduced at TO, T1 and T2 (p ＜ 0.05). In this study, the older age (≥50 y/o), female sex, experience of removable denture, only-mandible and zygoma implant involved patients tended to be associated with better OHRQoL. Smokers were directly linked to poor OHRQoL at T1 and T2 (p ＜ 0.05), and the design of definitive prostheses were not associated with OHRQoL. Conclusion: All-on-4 concept immediate functional prostheses were effective in improving OHRQoL during interim and definitive stages.
