癌症病患之植入式人工血管錯位合併內頸靜脈栓塞-病例報告

Malposition of Subcutaneous Implantable Intravenous Infusion Port Combined with Internal Jugular Vein Thrombosis in Oncology Patients-A Case Report

摘要


皮下置入型之靜脈導管爲癌症病人接受化學治療時例行手術放置的長期性靜導管,大部分的人工血管放置位爲鎖骨下靜脈。這種人工血管系統提供了更有效益,更安全的血管路徑,不過仍然有極少數的併發症會發生,例如感染、導管錯位、栓塞。導管錯位的臨床症狀有時候會產生疼痛感,也會增加感染和栓塞的機會,甚至會因爲藥物擴散至顱内導致神經學上的變化。這類型的併發症通常是和植入手術的方式,後續人工血管照護或維持有關。我們提出了癌症病人化療人工血管錯置於内頸靜脈並且發生靜脈栓塞罕見併發症之病例報告,並且根據回顧文獻上的記錄,針對發生的原因與處置的方式做進一步討論。

Subcutaneous implantable intravenous infusion port implantation is a standard surgical procedure in oncology patients receiving chemotherapy to build long-tern venous access. Most common site of placement is subclavian vein. This device provides a safe and effective venous access but there is still some limited complication rate, such as infection, malpositionb of the catheter, thrombosis. Clinical presentation of symptomatic Malposition of the catheter sometimes is pain, and moreover it even causes neurological complication resulted from cranial infusion of drugs. We provided an unusual case report representing internal jugular vein thrombosis due to malposition of catheter for chemotherapy in an oncology patient. Review of literature, factors related to this complication and clinical management will be discussed in this article.
