The Detection of the Posterior Superior Alveolar Artery and the Lateral Bone Thickness of Maxillary Sinus-A Three-Dimensional Cone-Beam Computed Tomography Study

後上齒槽動脈與上顎竇側壁骨厚度:三度空間錐狀束電腦斷層研究

The present study aimed firstly to investigate the prevalence of the posterior superior alveolar artery (PSAA), and secondly the relationship between the lateral bone thickness of maxillary sinus and the presence of PSAA in cone-beam computed tomography (CBCT) images. The presence/absence of the PSAA in the lateral wall of the maxillary sinus, and the anastomosis paths, the distances from PSAA to sinus floor, to alveolar crest and to mid-palate plane and the lateral bone thickness of maxillary sinus at different levels (3 mm, 5 mm and 10 mm from sinus floor) were documented. Total 230 sinuses were included and the overall PSAA prevalence in CBCT images was high (86.5%), yet varied at different tooth sites (57.6%, 35.8%, 47.8%, 81.2% at 1st premolar, 2nd premolar, 1st molar, and 2nd molar sites, respectively). The mean lateral wall thickness of maxillary sinuses was thinnest at the 2nd molar areas. The correlation between the prevalence of PSAA and the lateral wall bone thickness of maxillary sinuses was moderately positive at 1st molar sites, while weakly positive at 2nd premolar and 2nd molar sites. Patients with detectable PSAA in CBCT images possessed thicker lateral wall of maxillary sinuses than patients without detectable PSAA.

本研究目的為研究於錐狀束電腦斷層影像中後上齒槽動脈盛行率及其與上顎竇側壁骨厚度之關係。後上齒槽動脈之有無、其路徑與其位置(距上顎竇底、齒槽骨脊及正中顎骨平面之距離)、與上顎竇側壁在不同高度之骨厚度皆於影像中測量並記錄。總共測量230個上顎竇,後上齒槽動脈整體盛行率相當高(86.5%),且於各牙位盛行率有較高的差異性(分別為第一小臼齒57.6%;第二小臼齒35.8%;第一大臼齒41.8%;第二大臼齒81.2%)。上顎竇側壁骨在第二大臼齒區有最薄的厚度。後上齒槽動脈之有無與上顎竇側壁骨厚度之相關性,在第一大臼齒區呈現中等正相關;在第二小臼齒及第二大臼齒區呈現弱正相關。在影像中可以偵測到後上齒槽動脈之病患比無法偵測之病患有較厚之上顎竇側壁骨厚度。
