The branchial cleft cysts, or benign lymphoepithelial cysts, are uncommon anomalies of the embryonic development. These anomalies of brachial arches could be manifested in cysts, sinus tracts, or cartilaginous remnants and surgical excision is the main treatment modality. We reported a 52-year-old male with swelling on the left side of the neck region for 2-3 months. CT scan showed that there is a single well-circumscribed oval hypoattenuating mass located in the left submandibular area and measured approximately 2.5 cm x 2.5 cm in size. Surgical excision with a transverse cervical incision was performed under general anesthesia and sent for histopathology examination. The histopathological report revealed a benign lymphoepithelial cyst of left neck, type I. There was no tumor recurrence until now. Here we had a literature review focus on diagnostic modalities and treatment of branchial cleft cyst.
