Application of Virtual Surgical Planning and 3D Printing Model in Zygomaticomaxillary Complex Fracture-A Case Report

摘要


The zygomaticomaxillary complex (ZMC) plays a key role in the structure, function, and esthetic appearance of the facial skeleton. It provides normal cheek contour and separates the orbital contents from the temporal fossa and the maxillary sinus. Conventional treatment of ZMC fracture aims to reestablish function and esthetics. However, virtual surgical planning can provide more precise and predictable treatment outcomes. We report a case of a 20-year-old man diagnosed with a fracture of the right maxillary sinus, ZMC, and abrasion of the right upper lip, chin, and limbs caused by a traffic accident. Virtual surgical planning, including computed tomography imaging, intraoral dental scanning, Dolphin software simulation, and 3D printing technology, was performed to determine the optimal function (occlusion) and esthetics (profile). Under general anesthesia, open reduction and internal fixation and intermaxillary fixation with a pre-bend low-profile mesh over the right ZMC, orbital floor, and 3D printed occlusal splint was performed. The postoperative follow-up demonstrated a satisfactory outcome without any complications.

顴顎骨複合體在面部骨骼的結構、功能及外觀上扮演重要的角色。它提供正常的臉頰輪廓且將眼框內容物與顳窩及上顎竇分開。傳統治療顴顎複合體骨折的方法致力於重建功能及外觀。虛擬手術設計能提供我們更精確且更可預期的治療結果。本病例為交通意外而被診斷為右上顎竇、顴上顎骨複合體骨折及右上唇、下巴及四肢擦傷的20歲男性。虛擬手術設計包含電腦斷層、口內掃描、Dolphin軟體規劃及3D列印科技皆被執行以達到最佳的咬合功能及外型美觀。我們在全身麻醉下,用依3D列印模型預先彎折的鈦網及3D列印之手術導引板在右側上頷骨複合體及眼框底進行了開放式復位內固定和頜間固定。術後追蹤良好且無併發症。
