垂直牙根斷裂的診斷及治療
The Diagnosis and Treatment of the Vertical Root Fracture
摘要
垂直牙根斷裂不易診斷。其症狀常像牙周疾病或失敗的根管治療而不具特殊性，而且發生的早期並無明顯的症狀可供診斷，常於發炎到某一階段時，才有比較特殊的X光放射影像。裂紋一旦與牙齦溝相通，口腔內各種細菌即可順著裂隙長驅直入破壞牙周組織，造成垂直性骨喪失甚至是牙周膜腫。 治療方面，手術曝露是確定診斷的主要方法，而拔除是常見的治療方式。本文探討臨床上依據哪些症狀及方法，可以幫助診斷及治療垂直牙根斷裂。

Vertical root fracture (VRF) presents a challenge to general practitioners in the diagnosis. It usually shows similar symptom as periodontal disease or failed root canal therapy. The initial course of VRF is without expressing obvious or particular symptom. The radiographic image could reveal VRF only if the inflammation results in the findings of periradicular lesion and its location. Once the crack communicate with crevicular gingival sulcus, the periodontal tissue will be easily damaged and conduce to vertical bone loss or periodontal abscess. Although we could also have a definite diagnosis of VRF by surgical exposure, most cases are referred to extraction. In this article, we will consider how to diagnose and treat VRF according to common symptoms of VRF and current available examining methods.
