Autotransplantation of tooth (ATT) is a reliable method with a good prognosis to substitute a hopeless tooth. The structure of maxillary sinus creates an anatomical limitation for ideal tooth transplantation. The complications from oro-antral communication (OAC) often occur after the extraction of the maxillary tooth. We represent a case who was received the autotransplantation of teeth combined with sinus repair and sinus lift and had a satisfactory result.The patient was a 25 year-old male and had severely decayed bilateral maxillary first molar. Because the patient had bilateral third molars at both side of the mouth, we planed the autotransplantation of teeth to restore extracted areas. An OAC ,about 6 mm in size, was noted at the apical area and the donor third molar was transferred to the recipient socket to repair the communication on right side. And on the left side, ATT combined with the operation of osteotome sinus lift was applied to overcome the compromised maxillary ridge.Two years after the operation, the transplanted tooth was healthy in periodontium with stable occlusion. Autogenous tooth ransplantation used for sinus repair or combined with sinus lift technique expand the indication of tooth transplantation, and restores the masticatory function fast and economically. It is an alternative option to restore the missing tooth at maxillary posterior area.
