Clinically, maxillary sinusitis might be caused by odontogenic infection, due to the close proximity of the roots of the maxillary posterior teeth and the maxillary sinus cavities. This case report presents the apical periodontitis of maxillary second premolar which induced maxillary sinus mucosal thickening. After nonsurgical endodontic treatments, the immediate improvement was found by the evaluation of conventional X ray. We review the prevalence, classification and treatment considerations of sinus mucosal thickening by cone-beam computed tomography, and the associations between periapical lesions and sinus mucosal thickening.
