矯正後廣泛性慢性牙周炎之牙周治療－病例報告

Periodontal Therapy of Diagnosed Generalized Chronic Periodontitis after Orthodontic Treatment - A Case Report

摘要

患者為一位24歲的女性，於2017年的五月首次來院就診。患者自述於2012～2015年間在牙科診所接受矯正治療，但在矯正器拆除後，患者感到牙齒動搖度增加，且牙齦時常出血，故來本院尋求全口牙周評估。患者否認全身性系統性疾病也沒有抽菸。口內檢查後發現多處牙齦腫脹、牙菌班牙結石堆積及牙周附連喪失大於5毫米，且在全口根尖片及咬翼片上呈現嚴重程度不等的骨破壞，合併上述檢查結果診斷為慢性牙周炎。初始治療計畫為牙周病第一階段治療。在非手術性治療結束後，患者的牙齦紅腫流血症狀獲得改善。後續安排患者接受右上第一大臼齒的牙周翻瓣清創手術，及上顎前牙舌側維持器固定並建議左下及右下智齒拔除。手術後恢復良好，現於門診持續追蹤觀察中。

This 24 year-old female patient visited our hospital in May, 2017. The patient stated that she received full mouth orthodontic treatment at the local dental clinic from 2012 to 2015. Tooth hypermobility and gingival bleeding were found after debonding, therefore she came to our hospital for full mouth periodontal evaluation. She denied all major systemic diseases without smoking habit. After clinical examination, generalized gingival swelling, plaque and calculus deposition were observed. Horizontal and vertical bony destruction was also noted on periapical radiographs and vertical bitewings. According to the clinical findings, she was diagnosed as generalized chronic periodontitis and the tentative treatment plan was periodontal phase I therapy. After nonsurgical treatment, gingival swelling subsided. The patient was arranged for periodontal surgery of tooth 16, extraction of teeth 38 and 48 and upper anterior lingual fixed retainer. Uneventful wound healing after the flap operation, and she still under regular follow-up in our dental clinic.
