Treatment of Gingival Recessions with the Modified Coronally Advanced Tunnel Technique in Conjunction with Subepithelial Connective Tissue Graft: A Case Report

摘要

Gingival recession can result in root sensitivity, root caries, esthetic concerns, and difficulty in oral hygiene maintenance. Many surgical techniques have been reported to treat gingival recessions through root coverage. Recently, the modified coronally advanced tunnel (MCAT) technique has been developed where an envelope-designed flap without incisions of the papilla is coronally positioned and combined with a connective tissue graft to increase complete root coverage percentage and increase keratinized tissue width. This article highlights the results of using MCAT in two cases on a single patient to achieve root coverage in not only a single severe type 2 gingival recession, but also multiple type 2 gingival recessions. Complete root coverage was achieved in both cases and the patient was satisfied with the result.

牙齦萎縮會導致牙根敏感、牙根齲齒、美學問題以及口腔衛生維護困難。有許多方式可以治療牙齦退縮，其中透過移植軟組織進行牙根覆蓋便是一種可行的方式。近年來，有許多的手術方式被提出可以合併上皮下結締組織移植體進行牙根覆蓋，除了可以增加牙根的覆蓋率並可以增加角化組織的寬度。本文將介紹使用隧道穿通術合併上皮下結締組織移植體的術式，以及一些臨床考量事項。此外提出一患者分別在單顆牙與多顆牙嚴重的第2型牙齦退縮的情形下，進行牙根覆蓋，術後患者的兩個部位不僅獲得牙根覆蓋的結果，患者也對該結果感到滿意。
