牙齒嚴重磨耗患者的全口重建：病例報告
Rehabilitation of Severe Worn Dentition: A Case Report
摘要
長時間後牙缺失會造成嚴重的咬合面磨耗，導致在牙齒外型及上下齒列空間的喪失。進行重建時除了咬合功能的恢復外，需同時考慮牙齒外型美觀上的恢復，並且需針對造成嚴重磨耗狀況的原因進行處理。本病例為65歲男性病患，因長時間缺牙導致牙齒磨損嚴重，且因之前製作之臨時固定義齒經常脫落因此前來求診。經口內及X光檢查，先給予臨時固定義齒恢復牙齒的外型，並在缺牙區以臨時活動義齒重建後牙咬合關係。經過臨時義齒的追蹤，觀察關節與咀嚼肌群並未出現不適與其他症狀後，依據提高後的咬合高度製作正式義齒。本病例探討牙齒嚴重磨耗的狀況，如何擬定治療計畫，恢復咬合高度與牙齒外型。義齒完成後於門診追蹤近一年半，患者滿意咬合功能及美觀，義齒狀況均穩定。

Loss of posterior teeth in the long run will cause severe tooth attrition, and can even cause change of the tooth morphology and loss of occlusal vertical dimension. Not only do we need to restore the morphology of the teeth and the occlusal function, but also need to eliminate the etiologies of the attrition when dentist try to rehabilitate the worn dentition. This case was a 65 years old male patient, who visited our department due to severe teeth attrition and dislodgement of his previous fabricated fixed prosthesis. After intra-oral and X-ray examination, we gave provisional denture to restore edentulous area and fixed temporary prosthesis for attrition teeth. After 3 months follow-up, patient can accept the elevated vertical dimension and didn't have any symptoms of temporomandibular joint. So finally we use this vertical dimension and occlusion for permanent prosthesis fabrication. After one and a half years follow-up, patient didn't report any problem and was satisfied with the prosthesis.
