摘要

In addition to the high success rate of osseointegration for anterior dental implant treatment, aesthetic considerations are also very important. After tooth extraction, the extraction socket underwent a series of metabolism, as well as the material and size of the implants are different from the root. It is very difficult for the implant treatment to achieve the same appearance as the natural teeth. Immediate implant placement and provisionalization (IIPP) was often used to establish dental implant aesthetics in recent years. However, under IIPP procedure merely, it is might unable to compensate the gingival recession and alveolar bone resorption after tooth extraction. Hence, gingival graft or/and bone graft could be essential in different stages of treatment to make up for the deficiency. Some other studies suggested combining with the modification of the abutment to achieve the aesthetic requirement. In this paper, a case of IIPP combined with the special designed modification of abutment was presented to achieve the aesthetic requirements and also to increase the gingival support.

美觀區的前牙植牙治療，除了要求高骨整合成功率外，美觀的考量也十分重要。拔牙後，拔牙窩經過一系列的新陳代謝變化，加上植牙後，植體的材料及大小與牙根的差異，將來植牙治療要達到與自然牙相同的外觀，就顯得十分具有挑戰性。拔牙後立即植牙（即拔即種）並且立即製作臨時假牙的術式，是近年來常被利用來建立植牙美學的治療方式，但僅僅單獨使用這樣的術式，可能還是無法補償因拔牙後牙齦的萎縮與齒槽骨的吸收。因此有可能在不同的治療階段，利用移植牙齦或是（加上）骨頭的填補來彌補不足。有些學者更建議利用修改支臺齒的型態與材料來引導牙齦的生長。本篇文章提出一例病例，利用拔牙後立即植牙並且立即製作臨時假牙的術式，加上特殊設計的支臺齒修改，來達到植牙美觀的要求，並且增加提供牙齦支持度。
