耳軟骨複合移植重建玻尿酸注射後鼻翼穿透性壞死－病例報告
Composite Graft Reconstruction for Through-and-Through Nasal Alar Defect after Hyaluronic Acid Injection-A Case Report
摘要
背景：玻尿酸注射目前已廣泛用在軟組織的填充。然而其有可能因為注射所導致的併發症，如血管栓塞，而造成災難性的結果。目的及目標：利用耳軟骨複合移植重建手術重建因玻尿酸注射所導致之穿透性鼻翼軟骨及皮膚缺損。材料及方法：一位經玻尿酸注射鼻頭造成鼻翼壞死的年輕女性，接受清創後發現鼻翼穿透性壞死。整形外科醫師應用自體耳軟骨複合移植重建手術重建鼻翼軟骨及皮膚缺損,術後並給予高壓氧治療。結果：病患術後6個月追蹤，鼻部外觀輪廓尚為對稱，僅有些微皮膚顏色不均及鼻孔上縮。結論：對於玻尿酸注射後所導致的穿透性鼻翼缺損，耳軟骨複合移植重建是一個單一階段且可行的手術方式。

Background:Hyaluronic acid filler injection is widely used for soft tissue augmentation. However, there could be disastrous results by complications such as direct vascular embolization.Aim and Objectives：This article presents the details of using concha composite graft to help surgeon to reconstruct through-and-through defect at nasal ala caused by injection necrosis of hyaluronic acid.Materials and Methods:A case of ischemic skin and cartilage necrosis over nasal ala after hyaluronic acid filler injection on nasal tip. Through-and-through defect was noted after debridement. Composite cutaneous cartilage graft harvested from concha was performed for reconstruction.Results:Following up for 6 months, the nasal profiles of the patient was successfully restored except for some skin color mismatch.Conclusion:Using conchal composite graft is a good choice for reconstruction of through-and-through defect over nasal ala.
