Masson's Tumor of the Hand-＂A Case Report and Literature Review＂

背景：梅森氏腫瘤，是血管內血管內皮細胞增生的一種疾病。雖然是罕見的軟組織腫瘤，在臨床上卻容易與惡性血管肉瘤混淆。目的及目標：我們分享一個病例其手掌尺側梅森氏血管內皮細胞乳突狀增生，經過手術將血管異常部分切除並無局部再發而且不影響任何手部功能，並相關文獻整理報告。材料及方法：我們報告一位18歲的男性患者，其尺側手掌及手腕有多顆疑似血管瘤的軟組織腫瘤，有壓痛並摸起來有硬塊感。核磁共振影像檢查顯示T2顯影T1不顯影與血管瘤相符，而病理切片檢查顯示為梅森氏血管瘤即血管內皮細胞乳突狀增生，所以我們針對手掌及手腕腫瘤做了切除手術。在術後的門診追蹤一年並無局部復發，病人的手部功能也未受影響。結果：病人術後住院康復時間很短，手部的功能及疤痕恢復良好，手術後追蹤三個月的期間病人相當滿意。結論：梅森氏血管瘤雖惟可以病理切片確診，是一種良性腫瘤可以以手術治療成功並移除血管內結石及改善症狀。手術常可以根除，但若為由血管畸型而來的例子，病例報告有局部復發的機率。

Background: Masson's tumor, also known as intravascular papillary endothelial hyperplasia is a rare vascular tumor. It is a benign lesion and often cured by surgical resection. Aim and objectives: We presented a case of multiple Masson's tumors arising on the medial aspect of palm and also reviewed the histological features and current literature regarding differential diagnosis and management. Materials and Methods: An 18-year-old male patient sustained several protruding soft tissue masses on the medial aspect of palm since childhood. The soft tissue masses enlarged slowly and became painful intermittently. Incisional biopsy revealed intravascular papillary endothelial hyperplasia. Magnetic resonance imaging (MRI) also showed vascular lesions at ventral and dorsal aspects of left hand and fingers. Subsequently, we performed total excision surgery of the vascular neoplasm. Histological pictures were also described in the article. After discharged from the hospital, we followed up this patient regularly in the out-patient clinic for one year. Photographs of lesion site were taken during out-patient visit as part of our clinical assessment and documentation. Results: The final pathology confirmed the diagnosis of intravascular papillary endothelial hyperplasia. After complete resection, there was no recurrence and the function of the hand was preserved. Conclusion: To our knowledge, Masson's tumor has been described as a solitary vascular tumor. Therefore, it was the first case report that represented multiple Masson's tumors arising on the hand. Although it may recur, surgical resection resulted in good control of the disease and also relieved patient's symptoms.
