Background: Traumatic defect of the big toe is common and reconstruction is essential for restoration of function. Medial plantar artery flap, free or pedicled, is a useful option for forefoot reconstruction, providing glabrous skin with minimal donor site morbidity. Objective: We report a case of right big toe pulp reconstruction with a free medial plantar artery flap in a 2-year old boy after traumatic amputation and the surgical modifications necessary to accommodate for the small calibre of the vessels in such a young patient. Material and Methods: 2-year old child with 3×2cm pulp defect in the right great toe. Reconstruction was performed with free medial plantar artery flap, using the dorsalis pedis artery and concomitant vein as recipient vessels. Results: Full flap survival and no functional deficit after 1 year follow up. Conclusion: Medial plantar artery flap is an option for pediatric toe reconstruction.
