中醫針灸處置遺漏針之改善專案

The Project of Decreasing the Missing Needle during Acupuncture Treatment

摘要


病人安全議題近年來受世界各國的重視,而針灸處置是中醫最常見的侵入性治療方式,故安全防護上更加重要。專案小組發現針灸處置照護過程中,以遺漏針的發生率佔最高,而針遺留在病人身上造成的後果,為針灸科病人安全的一大威脅。本專案藉由現況分析遺漏針之原因,重新審查針灸標準作業流程、加強針灸前後護理衛教、設計人形圖記錄單張,並創新製作出出針輔助器、修訂中醫針灸護理記錄單、執行針灸護理品質監控、定期舉辦在職教育、並與醫師溝通討論治療中補針的記錄方式等七項改善措施,希望能降低異常事件發生,經實施3個月後,遺漏針發生率由改善前3.3%降至0.29%,幅度達91.22%,期望透過本專案之經驗及創新輔具的應用,將此過程分享作為同儕改善的參考建議。

Patient safety issues subject to the global attention in recent year. Acupuncture is the most common TCM inverse treatment, so the safety become more important. Our group found that the most common accident occur was leaving needles in the patient's body. Such accident will become a major treat to patient safety. The project analyzed the current situation to understand the reasons of leaving needles in the patient's body. Seven steps were used to improve the situation: re-exam the acupuncture standard operating procedure, enhance the health care education before and after acupuncture treatment, designed humanoid record sheet and created needle taking aid, revised acupuncture nursing record sheet, carried out the nursing care quality monitoring, conducted regular in-service education, and coordinated with physicians about record treatment. After three months, the incidence rate of leaving needle in patient's body was reduced from 3.3% to 0.29%. The declining rate was 91.22%. The project experience and innovative appliances can be used as peer reference recommendations for improvement.
