Case Report: Exogenous Estrogen Toxicosis in a Dog

病例報告：犬之外源性動情素中毒

摘要

天然或合成動情素在臨床上應用於雌犬之人工流產，或治療假懷孕及結紮後雌犬之尿失禁現象。於公犬則用於減緩攝護腺肥大或肛門腺瘤之症狀。於犬，此藥物可產生之副作用，包括骨髓抑制、出血症候群、子宮蓄膿症等。一隻五歲貴賓雌犬經肌肉注射ECP（estradiol cyclopentyl propionate 5 mg/次）3次（第0，3，10天）治療假懷孕後，逐漸出現皮下出血斑和陰部分泌物，於第一次注射之第15天被診斷有子宮蓄膿症而施以卵巢子宮切除術（OHE）。此病例於第20天（OHE後第5天）轉診至台大農學院附設家畜醫院。當天之血液學檢查發現有中度貧血（PCV 20.3%），血小板減少（17,000/pL）和嚴重白血球增多症（111,400/μL)，白血球總數卻於第27天嚴重的下降到5,160/μL。此病例於第20天至第41天才投以抗生素，止血劑和維他命－礦物質等。於第41天檢查得到正常血球相，並一直持續至治療停止後（第50天）。

An estrogen toxicosis in a 5 years old, 2.6 kg toy-poodle bitch which had been treated for clinical pseudopregnancy with estradiol cyclopentyl propionate (ECP , 5 mg/each time) for 3 times (day 0, 3, 10) was diagnosed. Bleeding tendencies occured gradually, in addition, pyometra was diagnosed and OHE (ovariohysterectomy) was performed on day 15. Normocytic hypochromic anemia with thrombocytopenia (17,000/μL) was recorded on day 20. Meanwhile, a severe leukocytosis (111,400/μL) on day 20 was followed by a precipitious drop to 5,160/μL on day 27. The dog was treated with antibiotics, vitamine-mineral supplements and hemostatics between day 20 and day 41. Normal hematologic values were obtained on day 41 and were obtained after the medication stopped (day 50).
