病例報告：犬外耳道惡性混合型耵聹腺瘤

本報告是一個犬外耳道惡性混合型耵聹腺瘤病例。患犬為一6歲齡的雄性喜樂蒂牧羊犬。因其長期存在有無法治療的雙耳膿性排出物、搖頭、抓耳等症狀，被送至國立中興大學附設獸醫教學醫院診療。該犬除上述症狀外，左耳外觀明顯腫大，並伴隨耳旁膿腫。二耳外耳道均被不規則形狀的組織團塊充塞。左耳道與右耳道之團塊組織分別以外科手術行全耳道切除術與耳道垂直部切除術摘除。切下之左耳道組織切面呈堅實的淡褐色團塊，右耳道之病變組織則包括黃白色堅實與柔軟的黃棕色組織。由組織病理學檢查可見二耳道病變組織均有慢性增殖性外耳炎，其中左耳道增生之耵聹腺多失去腺體結構，且其細胞發生退行發育現象，屬於腺癌的特徵。左耳道組織病變尚可見膠原纖維增生、多處軟骨樣組織化生區等，因此診斷為犬外耳道惡性混合型耵聹腺瘤。該犬術後復原良好，腫瘤經手術切除後一年未有復發情形。

This is a case report of malignant mixed ceruminous gland tumor of the external ear canal in a 6-year-old male Shetland sheepdog. The dog was presented to Veterinary Teaching Hospital at National Chung Hsing University with a history of chronic, incurable bilateral purulent discharge from the ear canals, head shaking, and ear scratching. In addition, it had para-aural abscess and marked swelling of the left ear canal. The lumen of both ear canals was completely blocked with irregular hyperplastic tissues. The left and right ear canals were resected with total and vertical ear canal ablation, respectively. The cut surface of the tissue mass from the left ear canal was firm and pale brownish, but that from the right ear was firm yellowish to soft brownish. Microscopically, both ears were affected with chronic proliferative otitis externa characterized by the hyperplasia of ceruminous gland with cystic dilation in the acini and ducts accompanied by acinopapillary projections in the lumen of the enlarged glands. Most of the hyperplastic glands in the left ear canal had lost their glandular structure and had cellular anaplasia, which are characteristic of the ceruminous gland adenocarcinoma. Moreover, there were fibrosis and, multifocal cartilage metaplasia. Therefore, the tumor in the left ear canal was diagnosed as a malignant mixed ceruminous gland tumor. The dog recovered uneventfully after surgery and there was no recurrence in a one-year follow-up period.
