上顎半邊截除患者顎補綴物的設計
MAXILLOFACIAL PROSTHESIS DESIGN IN A CASE AFTER HEMIMAXILLECTOMY
摘要
一位於上顎右側罹急腺樣囊狀癌（adenoid cystic carcino-ma）的患者，經施行上顎半邊截除手術六個月後要求顎顏補綴複建。傳統上此種補綴物的製作因原有組織已蕩然無存且重點多針對閉孔器的設計，而對於患者之咬合平面及義齒精膌外形的複建多半僅模擬對側殘餘部分加以雕刻，不但相當費時且效果不彰。本篇報告乃作者利用患者手術前的完整齒模將其牙齒、咬合平面、齒槽膌與膠部忠實地複製。患者不但能於最短時間內重新恢複正確之發音且在咀嚼、吞嚥及美觀上都有立竿見影的效果。

A prosthetic appliance which duplicates the presurgical appearance and contour may be more acceptable to the patient. We presented a technique for fabrication of a permanent obturator in which the false palate, false ridge , teeth and occlusal plane were correctly reproduced. This technique permits replacement of all teeth at the same position and of the same shape as were presented preoperatively. In addition, the new prosthetic obturator provides an environment for the tongue similar to the original condition, thus may simplify the denture learning time, phonation and deglutition.
