使用前列腺素E1對如動脈導管縮窄即可引起危急症狀之重症先天心性心臟嬰兒,提供新的緩症輕狀之重要方法。著者提供三病例前列腺素E1之經驗報告,證實前列腺素E1確可使這些病例之動脈導管迅速擴張而緩輕臨床症狀,因而為外科手術治療爭取更多有利的時間。
Persistent patency of the ductus arteriosus (PDA) 15 necessary for survival in certain types of congenital heart disease. Neonates with pulmonary atresia or critical pulmonary stenosis are almost entirely dependent upon the ductus for the maintenance of pulmonary blood flow. Interruption of the aortic arch or critical coarctation of the aorta may require a PDA for adequate blood flow to the lower half of the body. A PDA may also benefit patients with tricuspid valve anomalies who have right to left atrial shunting and restricted pulmonary blood flow. In patients with transposition of the great arteries and persistently severe hypoxemia after a balloon atrial septostomy, opening of the ductus may increase pulmonary blood flow further and thus enhance intracardiac mixing . Muscle tone of the fetal ductus arteriosus appears to be controlled in part by the endogenous production of prostaglandin compounds.(1-4) The ductus arteriosus in preterm infants can be pharmacologically closed by inhibitors of prostaglandin synthesis such as indomethacin.(5) Prostaglandin E1 (PGE1 ) has been shown to dilate the ductus arteriosus in humans. (6-10) This pharmacologic agent has been used in clinical trials in selected centers, and it has been recently approved for general use by the American Food and Drug Administration . The purpose of this report 15 to present several representative cases admit-ted to Arkansas Children’s Hospital where PGE1 appeared to be a life-saving drug, and to make suggestions for the use of PGE1 in hospital nurseries in Taiwan.
