Alterations of Nucleolar Organizer Regions of Smooth Muscle Cells in Bile Duct Following Biliary Ligation in Rat

老鼠膽管結紮後膽管平滑肌細胞核仁區之變化

摘要


膽管阻塞時,在臨床上其膽管會發生擴張的現象。本研究目的即以膽管結紮後,觀察膽管壁內平滑肌細胞之增大及增生現象,期待了解膽管擴張之膽管壁細胞組織之來源。本實驗以雄鼠 27 隻分成四組;第一組為對照組( n = 7 ) ,第二組( n=6 )為膽管結紮四天後再取其擴張膽管做組切片,第三組( n = 7 )為結紮七天及第四組為結紮十四天後再取其標本。經薄切片後,再以銀染色觀察平滑肌細胞之大小,數目及其核仁數目,且利用腦影相分析儀計算細胞之大小及數目加以分析。其平滑肌細胞之切片最大寬度分別為 2 . 82±0.07 陣, 3.05±0 . 06。 m , 4.53±0.l0pm 及 4.89±0.12 陣而其膽管平滑股細胞之數目每 0 . lmm2 膽管壁增加情形,其第一至第四組分別為 9 . 08±0 . 48 , 12 . 30±1.25 , 25.54±1.15及28 . 23±1 . 51 之增加,而其細胞核仁( AgNoR )數目亦分別為 2 . 06±0.02 , 2.39±0.03 , 2 . 55±0.03 及 2.42 ±0 . 030 由本實驗可知在膽管結紮時,其膽管壁內平滑肌細胞數目及體積均為增加而造成膽管擴大之組織來源,且由核仁區數之增加而確立其增生現像,然而原本平靜細胞為何因素而增大及增生是有待進一步之研究的。

Clinical experience indicatçs that bile duct dilatation is often associated with patients presenting with cholestasis. The purpose of this study is to propose a hypothesis that such a biliary dilatation, resulting from extra-hepatic. obstruction, partially contributes to the rapidly occurring hypertrophy and hyperplasia of the smooth muscle cells in the bile duct; and that such an alteration could possibly lead to the development of neoplastic tissue. Twenty seven male Wistar rats were divided into 4 groups: Control Group (n7) with sham operation, Day 4 Group (n6) with common bile duct ligated for 4 days, Day 7 Group (n=7) with common bile duct ligated for 7 days, and Day 14 Group (n=7) with common bile duct ligated for 14 days. Complete extrahepatic biliary obstruction was induced in Wistar rats by distal ligation of the common bile duct (CBD). The CBD specimens were stained by H & E and argyrophilic techniques (AgNOR method), and the smooth muscle cells were identified and studied with light microscopy and an image analyzer. To determine maximun width of cell size, the widths of smooth muscle cells were found to be 2.82±0.07 jim, 3.05±0.06 jim, 4.53±0.10 jim, and 4.89±0.12 Jim (Mean±S.E.) for the Control Group, Day 4 Group, Day 7 Group and Day 14 Group, respectively. For the smooth muscle cell number, the results were 9. 08±0.48 cells per 0.1 mm2 for the Control Group, 12.30± 1.25 cells for the Day 4 Group, 25.54± 1.18 cells for the Day 7 Group and 28.23± 1.51 cells for the Day 14 Group. These results were statistically significant for both cell size and cell number when compared to the control group. Results from Argyrophilic Nucleolar Organizer Regions (AgNOR) were 2.06±0.02, 2.39±0.03, 2.58±0.03 and 2.42±0.03 for the Control Group and Day 4, 7 and 14 groups, respectively. The results were also statistically significant when compared to the control group. These results provided the initial hypothesis that hyperplasia and hypertrophy of CBD smooth muscle cells have occurred, but the clinical significance of such a change remained to be further investigated.
