The Impact of Caring Experiences of the Patients with Borderline Personality Disorder on Psychiatric Nurses

摘要

本質性研究的目的在探討精神科護理人員照區邊緣性人格(bordering personality disorder, BPD)住院患者之經驗對其個人的影響。本研究以立意取樣，針對北市某一精神專科醫院有照顧過BPD病患之護理人員，採半結本式會談指引，與個案面對面進行回溯性的錄音訪談。再以質性內容分析法(qualitative content analysis)有系統地深入分析訪談內容。結果有13位女性參與本研究，86%年齡介於26-35齡；38%具有護理師資格；54%的精神科護理工作年資為5-7年；69%曾照顧過3-4呈具BPD特質住院患者。研究亦發現，護理人員照顧此類病人的經驗對其個人的影響，包括：1)增加自已對病人特質及專業知識的認知；2)對護理工作動機有正或負向影響；3)改變生活中與人互動的態度；4)更加珍惜自已擁有的事物；以及5)增加護士的自我瞭解及成長等。本研究結果有助於幫助精神科護理人員，更深入瞭解此類病患的特質，並增加照顧此類病患的動機與準備度，進而提昇對邊緣性人格住院患者的照顧品質，並增加護理人員的成就感。

The purpose of this qualitative study was to explore the subjective perceptions of the impact of caring with the hospitalized patients with borderline personality disorder (BPD) on psychiatric nurses. A purposive sample of psychiatric nurses with wxperiences of caring with at leats one hospitalized BPD patient was obtained at one leading psychiatric hospital in northern Taiwan. A semi-structured interview guide and audiotaped recording skill ws employed, and data were analyzed by qualitative content analysis. Thirteen female nurese participated in this project. Eighty percent of them were aged between 26-35 (mean±SD=31.77±4.92). Thirty-eight percent of nurses wre registered professional nurses; 54% worked as psychiatric nurses for 5-7 years; and 69% ever took care of 3-7 subjects with BPD. Several impacts of the caring experiences on nurses were identified. They are as follows: (1) becoming more knwoledgeable about the BPD patients’ characteristics and the related nursing care;(2) having positive or negative motivatons of nursing profession; (3) changing nurses’ attitudes of interpersonal relationship; (4) valuing nurses’ personal belongs; and (5) enhancing nurses’ understanding adnd facilitating their personal growth. The result of this study may empower psychiatric nurses to better understand the BPD patients, and to increase nurses’ motivation of caring them. It may also enhance the quality care for these patients and the nurses’ sense of satisfaction.
