This research is sponsored by the Department of Health to promote the implementation of discharge planning for the hospitalized patients with tracheotomy. The purpose of this planning is to ensure that the patients can be discharged from the hospital or transferred to a long term care institution after their serious symtoms are relieved, and obtain a good care thereafter. 46 patients in this experimental study were discharged successfully from the hospital after implementing the discharge planning by the nursing staff. Among them, 21.7% patients returned home, and the others were transferred to the Veterans' home, the nursing home, or local hospitals. The results show that the family support has the most significant effect on the patient's choice of resident place after being discharged from the hospital. The most satisfactory service is the consulting service by the telephone, and the most demanding service for patients' families is to teach them the care technique. Four weeks after being discharged from the hospital, 80% patients could keep the same or have improving activity capability and physical situation.
