摘要

本研究旨在測試「麻醉恢復期病人氣管內管拔除標準」之信效度，期望由此實證護理結果，能運用於術後麻醉恢復期病人的臨床照護。本研究為類實驗研究設計，採立意取樣，以台北某醫學中心麻醉恢復室全身麻醉進行手術之病人為對象共418位，分實驗、對照二組進行。以SPSS軟體之次數分配、百分比、平均值、標準差、X^2、t-test、迴歸分析進行資料統計分析。研究結果顯示1.依據拔管標準執行拔管動作皆可安全拔管；2.二組病人在等待拔管時間上有顯著差異性；3.護理人員依據「拔管流程」及「病人氣管內管拔管評估表」執行拔管，無論在節省時間、降低成本、專業參與和成長、病人安全等認同度普遍皆高。此實證研究結果作為臨床照護此類病人準則、以縮短病人等待拔管時間，增加手術病人住院滿意度，提升護理照護品質為目標。

The purpose of this study was to examine the reliability and validity for the extubation of patients in postanesthesia care unit. 418 patients fittins the sampling criteria underwent general anesthesia. According to the Extubation Protocol, nurses evaluated very carefully and completed the extubation directly and successfully. Based on the nursing care model of the recovery room for this empirical research, we build up the systematic and clinical-service-oriented standard. The clinical nurses could complete the extubation process successfully, plus reducing intubation time. This nursing model can be applied and change the decision making of the PACU. Furthermore, it will increase the satisfaction rate, and improve the quality of nursing care, providing an evidence-based healthcare to post-operative patients.
