運用Watson關懷理論照顧一位腦中風病人之護理經驗

Nursing Experience in Utilizing Watson's Theory for a Patient with Stroke

摘要


本文是運用Watson關懷理論協助一位無法接受腦中風造成肢體活動障礙產生無望感的病人,以期能協助找回希望。筆者於民國2008年4月2日至5月9日照顧期間透過直接照護,運用觀察、會談及Gordon十一項功能性健康型態評估等技巧收集資料,確立病人住院期間最主要的健康問題為:無望感、肢體活動障礙、自我照顧能力缺失。於護理過程中運用Watson關懷理論為基礎,與病人建立治療性關係,提供臨床照護促進身體上的舒適,採接納及傾聽的方式,促進病人表達自我的感受,適時給予支持與鼓勵,引導了解及接納自己,協助渡過無望感的危機,進而增強復健訓練,發揮最大的身體功能,改善自我照顧的能力,克服生活障礙,使病人回歸正常生活。期望此護理經驗有助於提升護理人員對腦中風病人出現無望感之照護。

This case report aimed at describing the nursing experience in caring for a patient with left hemiplegia after stroke. Three major health problems were identified including the feeling of hopelessness, disturbance of physical activity and impaired self-care through observation at interviews, therapeutic relationship, physical examination and Gordon's 11-item functional health pattern assessment guide. Nursing intervention based on Watson's theory in caring was used in this case. After the nursing intervention, the patient overcame these health problems, became more motivated in rehabilitation and improved in the ability of self-care. We hoped this experience might provide other nurses with information about caring for stroke patients in the future.
