一位胸髓損傷外籍勞工之護理經驗

本報告探討一位胸髓損傷之外籍勞工病人，因意外導致胸髓損傷接受神經修復術後，疾病適應的護理過程。筆者經由直接照護、觀察及與個案會談等方式，運用同理心、陪伴和關懷建立了良好的護病關係，並運用羅氏適應模式，完整的收集評估資料；確定病人的主要健康問題有：肢體活動障礙、便秘、高危險性感染、個人因應能力失調等。因為此個案為外籍人士，各項護理措施的設計與執行，必須考慮病人本身語言能力、知識層級、學習態度，並須兼顧其國家的風俗習慣來做完整的護理計畫。過程中運用護理人員的專業知識和技能，針對此外籍背景，提供個別性護理措施，協助並了解外籍勞工在不同文化背景下面臨疾病的調適過程。謹將此一護理經驗提供大家分享，希對日後照護此類病人能有所助益。

This case report aimed at discussing a nursing process adopted for a foreign laborer with thoracic spinal injury after a nerve repair operation. During the nursing process, the author collected information through direct care, observations and interviews based on Roy's Adaptation Model. Primary health problems identified included: physical activity barriers, constipation, potential infection risk, and individual coping disorders. During the nursing period, we applied individual interventions according to the laborer's foreign background, and used professional knowledge and skill to strengthen his ability to adjust and adapt to his disease. I hope our experience would be helpful in the future when taking care of such foreign patients.
