達文西手術系統在婦科手術之應用

摘要

微創手術是目前婦科手術的趨勢，優點為恢復迅速、美觀及疼痛程度較為輕微。但是傳統二維影像系統及手術器械之限制，很多複雜手術如癌症、沾黏、肌瘤、不孕症等，均無法以傳統腹腔鏡完成。達文西手術系統提供立體三維影像及器械活動無死角的功能，可以克服傳統腹腔鏡的限制，提供外科醫師更優質、精密、安全，有效的協助。在婦科癌症手術之應用中，手術出血量、輸血比率、術後住院天數、手術中併發症及改成傳統手術的比率都優於傳統腹腔鏡手術。本文旨在介紹達文西手術系統應用於婦科手術的優勢，內容包括：達文西手術系統的介紹、婦科手術應用、手術室配置、病人準備、孔道定位與對位等，並提供我們參與手術照護之經驗，可作為手術室相關護理人員照護之參考。

Minimally invasive surgery is the current trend towards abdominal operation with many advantages, such as quick recuperation, good cosmetic outcome, as well as minor discomfort. Despite significant advances in conventional laparoscopic techniques, the majority of complex gynecological procedures are still performed through major abdominal incisions including oncologic surgery and adhesiolysis. The da Vinci Surgical System offers several advantages over the traditional laparoscopic method. The da Vinci surgical system was approved for use in gynecology in April 2005 and many procedures were translated to a robotic approach in USA. There is good evidence that robotic surgery facilitates laparoscopic procedure with equivalent, if not, shorter operative time, comparable surgical outcomes, shorter hospital stay, and fewer major complications than with surgeries using the laparotomy approach. Finally, we also discuss the setting and nursing care experiences with the da Vinci surgical system.
