一位初次罹患愛滋病個案的護理經驗
Nursing Experience of an Acquired Immunodeficiency Syndrome Patient
摘要
本文乃是照顧一位初次診斷愛滋病個案，從被告知罹病開始的焦慮到接受疾病的護理過程。護理期間2011年3月8日至4月1日，筆者運用Gordon十一項健康功能評估為工具，藉由會談、身體評估及理學檢查等方式收集資料及了解個案感受、需求，並運用敏銳觀察力及評估能力，確立個案主要健康問題有：活動無耐力、腹瀉、無望感。筆者藉由提供個別性的護理措施，如：提供氧氣、指導漸進式活動方式，緩解呼吸喘症狀維持住院期間血氧濃度於90﹪以上；教導皮膚清潔維持皮膚完整，促進生理舒適、提供飲食衛教及配合醫療處置，改善腹瀉問題；運用關懷、同理心建立信任關係及治療關係，提供疾病相關衛教，增加個案對疾病的認知，協助個案勇敢面對疾病，重拾對生命的希望。初次診斷愛滋病的病人，除須面對生理產生的不適症狀外，更需面對心理的矛盾與掙扎。筆者於照護過程運用主動關心個案，與個案建立信任感及治療性人際關係，適時提供支持及鼓勵，增進個案對治療的信心，進而使個案可面對疾病，走出愛滋病的陰霾。

This article is a nursing care experience of a patient who was diagnosed with acquired immunodeficiency syndrome (AIDS). Data was collected through interview, physical examination assessment. According to Gordon's 11-item functional health assessment, patient's health problems were (1) activity intolerance, (2) diarrhea, and (3) hopelessness. Nursing interventions included helping activities progressive methods to support the saturation above 90% during the admission, promotion of physical comfort, diet health education, establishment of a trusting relationship, and assistance to disease awareness, and confrontation of disease. The author provided encouragement, spiritual support, and establishment of interpersonal relationship to help the patient face the problems encountered by people infected with HIV.
