摘要

克隆氏症是終其一身反覆發作之疾病，飲食管理為自我照護之重要預防措施。本文探討一位女性外籍勞工罹患克隆氏症之護理經驗，照護期間自2014年8月18日至2014年8月27日，筆者運用Gordon 11項健康功能型態及觀察、會談、傾聽、直接照護進行護理評估，確立個案有急性疼痛、焦慮、個人因應能力失調等健康問題。個案除要面對疾病的治療，由於語言及文化不同，對於未來選擇台灣飲食的管理學習更顯困難。筆者運用食物圖卡說明建議及禁忌飲食，由翻譯人員與仲介協助，輔助翻譯軟體協助其瞭解飲食日記記錄的重要性及記錄方式，促進其對台灣飲食的選擇及飲食管理，協助降低個案焦慮，最終提升其疾病適應與自我照護之能力。

Crohn's disease is a whole-body disorder that involves lifelong fl are ups. Dietary management is a crucial means of symptom prevention and self-care among patients diagnosed with this disease. This article describes the nursing care experience of a female migrant worker with Crohn's disease. The nursing care period was between August 18 and 27, 2014. We employed Gordon's 11 functional health patterns assessment as a method of analysis and used observations and interviews to collect data. The patient presented with 3 health problems: acute pain, anxiety, and dysfunctional coping ability. This patient faced several difficulties in learning dietary management for this condition in Taiwan, namely the possibility of reoccurrence of this disease, the language barrier, and psychological pressure from culture shock. We used food picture cards and called on interpreters and agents to explain diet recommendations and prohibitions. Translation software assisted in the explanation of the importance and correct usage of a food diary. These techniques improved the dietary management capability of the patient through lowering anxiety and thoroughly explaining dietary choices and planning in Taiwan. Finally, we improved the ability of the patient to adapt and to maintain self-care.
