二度燒傷病人服用精胺酸對住院天數之實證應用成效

Effectiveness of Enteral Arginine on Length of Stay in Patients with Second-degree Burn Injury

摘要


當燒傷深度達二級真皮層以下,早日進食及補充足夠營養,可促進傷口癒合,以利早期復原,縮短住院天數。本研究目的為二度燒傷病人服用精胺酸對住院天數的實證應用成效,屬於治療型問題。經文獻搜尋、內容評讀、研究方法評分和臨床運用後,發現二度燒傷病人服用精胺酸與未服用者在加護病房住院天數(19.13±12.32 vs. 23.06±16.03)及總住院天數(29.13±18.86 vs. 31.60±18.37),統計上皆無顯著差異,但可縮短4天加護病房住院天數,每日住院費用約省13,000至14,000元。未來可再進行實驗性研究,確認二度燒傷病人服用精胺酸之效果,精進實證照護措施之應用。

When the burn depth is deep to dermis layer, early feeding and sufficient nutrition supply can promote wound healing, facilitate early recovery, and shorten the length of hospital stay. This empirical study investigated the therapeutic issue of whether arginine supplement administration to burn patients can shorten hospita stay. Based on findings from the literature, the Jadad score, and clinical applications, administration of arginine supplement to burn patients did not significantly affect the outcome in terms of number of days of ICU hospitalization (19.13 ± 12.32 vs. 23.06 ± 16.03) or the total number of days of hospitalization (29.13 ± 18.86 vs. 31.60 ± 18.37) compared with burn patients not administered arginine supplement. However, the number of days of ICU hospitalization was shortened by approximately one week, saving approximately NTD 13,000- 14,000 per day in hospitalization expenses. In the future, an experimental study can be conducted to confirm the effect of taking arginine in patients with second degree burn injury and to improve the application of empirical care measures.
