摘要

乳房對女性而言，是身體外觀及性別的重要象徵。本文為描述一位女性乳癌病人行乳房重建手術後，歷經生理改變造成心理衝擊的護理經驗。護理期間自2018年4月27日至5月5日，藉由直接照護、身體評估、病歷查閱及會談方式收集資料，以Gordon十一項健康型態評估工具進行整體性評估，發現個案有潛在危險性感染、焦慮及身體心像改變的健康問題。照護期間教導居家傷口照護方式、提供乳癌衛教資料、勵志讀物、安排乳癌病友經驗分享及鼓勵參與乳癌協會、Line群組分享，減輕焦慮，進而適應乳房重建手術後身體的改變，並儘早重返職場；運用關懷及傾聽方式，鼓勵說出身體心像改變之感受，使其重拾信心並以正向的態度接受疾病及治療。藉此次乳癌病人行乳房重建手術後照護的經驗，提供日後類似病人的照護建議與參考。

This article explores the nursing experience of caring for a woman with breast cancer undergoing breast reconstruction surgery. The period of nursing care was from April 27 to May 5, 2018. Through personal medical data collection, physical assessment, and interview records, Gordon's 11 functional health patterns evaluation showed that the patient's problems were risk of infection, anxiety, and body image disturbance. During the period of caring, the author performed wound care and counseled the patient and her caregiver regarding the methods of caring. Additionally, health education leaflets, inspirational books, and support group access were provided to relieve anxiety, and the patient was encouraged to express her feelings. The patient regained her confidence and positive attitudes and adapted to her new body image after breast reconstruction surgery and returned to the workplace. The results of this nursing experience can provide colleagues with a case reference for similar patients.
