A Nursing Experience of Caring for a Sexually Assaulted Teenage Girl from a High-Risk Family

摘要

本文描述一位高風險家庭青少女遭受性侵之急診護理經驗，病人為16歲青少女，主訴與母親爭吵後離家，在外與男友碰面，卻遭男友在暗巷強迫性侵，由同學的家屬協助至本院就醫，至急診時無助哭泣引發撰寫動機。護理期間為2017年7月9日15:10－21:30，運用關懷技巧及傾聽收集資料，會談過程中發現病人父母離異，案母有吸毒前科，情緒不穩，對病人長期疏忽管教屬於高風險家庭，運用五大層面評估確認有強暴創傷症候群、無望感、危害性家庭因應能力等健康問題。醫療人員與社工共同提供相關社會資源並協助安置並給予支持，後續透過電訪追蹤皆有定期回診且逐漸恢復正常生活。希望能藉由此病人報告經驗，提供護理同仁日後照護類似病人之參考。

This article describes a nursing experience of caring for a teenager girl from a high-risk family and who was sexually assaulted. The patient was a 16-year-old girl who left her home after having an argument with her mother. The girl was raped after meeting her violent boyfriend outside her home. The family of one of her classmates accompanied her to our emergency department, where she received a physical examination. The period of care was between 15:10 and 21:30 on July 9, 2017. Information was collected by carefully listening to her during the interview, and she was discovered to be from a high-risk family because her parents were divorced and her mother abused drugs and had mood instability. Five-level assessment confirmed that the patient had rape trauma syndrome, hopelessness, and harmfulness in coping with familial responsibilities. The author collaborated with social workers for resource allocation, follow up, and finally, to help the patient accept herself, face her problem, and boost her confidence. This case report may provide some valuable information for clinical nursing staff who encounter similar cases.
