腸道管灌系統的細菌感染分析

Bacterial Contamination of Feeding Bags in Enteral Nutritional System

摘要


本研究乃針對使用連續性灌食袋之病人,觀察因懸掛步驟而引起的污染率及腹瀉的次數,並就其結果探討其相關性。本實驗設計以下三組以作比較: A組是使用新袋子,每天開袋子次數6次。 B組也是使用新袋子,每天開袋子次數則是10次。 C組則是使用曾經用過的袋子,每天開袋子次數是6次。 這些灌食袋經過24小時的連續性灌食後,測得總菌數超過5×10^(-4)個/ML的則為不合格,有污染。在A組15個袋子中有9個有污染,污染率佔60%。B組20個中有19個有污染,佔95%。C組2個中有19個有污染,佔95%。其中C組中有2個病人發生腹瀉的情形。而A組與B組及A組與C組的污染率經卡方分析檢定(Chi-Square Test)比較是有意義的。 實驗的結果是:1.管灌病人所使用的連續性灌食袋,經24小時的連續性灌食後,有普遍受污染的情形。2.開袋子的次數少及使用新袋子有較少的污染率。3.這些受污染的連續性灌食袋與病人的腹瀉無直接相關性。

This study was designed to evaluate the extent of bacterial contamination of continuous enteral feeding bags in clinical use and to observe the contamination rate and diarrhea frequency resulted from hanging step, and to find the correlation between them. Three groups were included in the experiment. Group A used new bags which were opened six times a day. Group B aslo used new bags, opened ten times a day. Reused bags were adapted in Group C and were opened six times a day. After 24 hrs of continuous feeding, if the number of colony forming units (cfu) per milliliter is more than 5×10^(-4) cfu/ml is considered as unacceptable. Nine out of 15 contaminations (60%) were found in Group A, 19 out of 20 (95%) were found in Group B and 19 out of 20 (95%) were found in Group C. Two patients of Group C had the discomfort of diarrhea. The comparison of contamination rate between Group A and B is as significant as between Group A and C by Chi-Square test. These results suggest that bacterial contamination of continuous enteral feeding was common in our Institution. The reduced frequency of opening enteral feeding bags along with the usage of new feeding bags resulted in less contamination rate. No direct relationship was observed between the contaminated continuous feeding bags and patients' diarrhea.
