The longevity of an osseointegrated implant is directly affected by maintenance of its osseous anchorage. Progressive marginal bone loss is detrimental to its survival. The purposes of this study were to examine the recording reproducibility and image homogeneity of serial radiographs taken with a modified alignment system and to investigate whether the device we developed significantly improves the reliability of radiographic measurements at the longitudinal peri-implant crestal bone level. The modified alignment system developed for recording serial intraoral radiographs included: (1) a modified surgical guide stent, (2) a modified commercially available film holder; (3) a newly developed technique based on mechanical fixation between the patient, the film holder, and the x-my tube; and (4) storage-phosphor computed radiography. Chi-square test and the F-test demonstrated that the geometric correspondence of pairs of radiographs taken with the modified alignment system was significantly better than that with the conventional method (p<0.05), and the standard deviation of measurement difference between pairs of radiographs taken with the modified alignment system was significantly lower compared to the conventional method (p<0.05). The use of the modified alignment system may provide dentists with an easier method to take images which show well-matched geometry for evaluating annual alveolar bone changes on serial radiographs.
